29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (d)(2) Machine or equipment shutdown. (d)(3) Machine or equipment isolation. (d)(4)(I) Lockout or tagout devices application. (d)(5) Stored Energy (d)(6) Verification of Isolation. a) On July 26, 2022, the employer did not ensure that each employee performing the servicing and Maintenance activity of gapping/adjustments on coiler 2 first shut the machine down((d)(2)) and isolated the machine's power sources ((d)(3)) in that the pneumatic system that powered the bottom wrapper arm was not locked out. As a result, an employee was exposed when the bottom pneumatic wrapper arm was activated. b) On or about July 19, 2022, the employer did not ensure that employees performing the servicing and Maintenance activity of changing the underhung table roll on PR3 Entry followed the lockout tagout procedures in that the spreader/side guides were not shut down and isolated. c) On or about July 25, 2022, the employer did not ensure that employees performing the servicing and Maintenance activity of Running the Polishing Sled, in the Coilers roll lines followed the lockout tagout procedures in that the rollers and slide guides were not shut down and isolated.
