29 CFR 1910.334(a)(3)(i): A flexible cord used with grounding type equipment did not contain an equipment grounding conductor: On or about July 16, 2022, employees were exposed to electrical shock hazards while working in and around Weld Station #4 because the attachment plug connecting the Airmaster Model CA30APE Pedestal Mounted Fan to the wall receptacle was missing the grounding pin.
