29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) On or about July 13, 2022, an employee was working under the Schelling cut-to-size saw, trouble shooting an air leak at the rest position of the saw carriage. The saw was not deenergized, locked out, and the saw carriage not blocked to prevent the saw from returning to the home position. This exposed the employee(s) to a struck-by and crushing hazard. The employer failed to implement energy control application steps as the machines were not isolated from all energy sources [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.
