29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (2) Machine or equipment shutdown. (3) Machine or equipment isolation. (4) Lockout or Tagout. (5) Stored energy. (6) Verification of isolation. (7) Release from lockout or tagout. a) On or about July 11, 2022, employees were exposed to machine hazards associated with the 400-ton Blow press during the setup activity of die changes. The employer failed to implement the energy control application steps as the 400-ton Blow press was not shut down or turned off to perform the setup activity (per 1910.147(d)(2-7) requirements). b) On or about July 11, 2022, employees were exposed to machine hazards associated with the 500-ton Komatsu press during the setup activity of die changes. The employer failed to implement the energy control application steps as the 500-ton Komatsu press was not shut down or turned off to perform the setup activity (per 1910.147(d)(2-7) requirements). c) On or about July 11, 2022, employees were exposed to machine hazards associated with the 300-ton Niagara press during the setup activity of die changes. The employer failed to implement the energy control application steps as the 300-ton Niagara press was not shut down or turned off to perform the setup activity (per 1910.147(d)(2-7) requirements).
