Tonnard Manufacturing Corporation, Bay 3 press area, on or about 07/06/2022 and times prior thereto: Employees perform service and maintenance on machinery including, but not limited to, hydraulic presses. Annual periodic inspections of the lockout tagout procedures were not conducted with authorized employees.
