29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition (a) Throughout the facility - On or about June 23, 2022 the employer did not perform regular inspections of factory where it would have been noticed that the gate at the R-74 rewinding machine was removed exposing employees to trip hazards.
