29 CFR 1910.28(b)(11)(ii): The employer must ensure: Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2) Stair width Enclosed One open side Two open sides Less than 44 inches (1.1 m). One stair rail system each open side. a) On May 24, 2022, the employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers was equipped with stair rail systems and hand rails as follows: in that the staircase located next to the End Matcher control panel had 2 open sides but only had 1 hand rail. b) On May 24, 2022, the employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers was equipped with stair rail systems and hand rails as follows: in that the staircase utilized to cross the conveyor to the second end matcher position had 2 open sides but had no hand rails.
