29 CFR 1926.1203(c): The employer who identified, or received notice of, a permit space and had not authorized employees it directs to work in that space did not take effective preventive measures to prevent those employees from entering that permit space, in addition to complying with all other applicable requirements of this standard: a) At the jobsite, where effective measures were not taken to prevent employees from entering the storm water lines, exposing employees to inhalation and engulfment hazards.
