29 CFR 1910.147(c)(5)(ii)(D):Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): At the workplace located at 4600 Waco Road, Columbia, Missouri 65202, employees were exposed to electrical shock and burn hazards associated with the application of LOTO as means of isolating hazardous stored energy. Equipment protection lockout devices that are utilized by the powerhouse and electrical department did not have a tag or any identification to show who had applied the lockout tagout device.
