29 CFR 1928.57(a)(6)(v): Lock out of electrical power was not conducted prior to performing maintenance or service on farmstead equipment: (a) On or about March 19, 2022, the employer assigned two workers to troubleshoot a Pit Manure Belt conveyor but did not ensure that the power source was disconnected and locked out prior to removing a guard. This condition exposed employees to hand and finger amputations. Opal Foods, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1928.57(a)(6)(v), which was contained in OSHA inspection number 1525247, citation number 1, item number 1 group letter B and was affirmed as final order on 10/14/2021, with respect to a workplace located at 9575 Weld County Road 73 Roggen, CO 80652.
