Queen Turret Pega-345 Punch Press Area - On or about January 20, 2022 and at times prior, the employer exposed employees to amputation hazards, in that the employer failed to perform maintenance or repairs on the machine's turret station, where a guard at the point of operation was missing while employees changed the tools that punch press metal sheets.
