29 CFR 1910.119(d)(3)(i)(E): Information pertaining to the equipment in the process did not include a ventilation system design: a) In the Machinery Room, on or about December 22, 2021 - The employer did not document ventilation system design basis, such as but not limited to applicable codes and standards, methods used to determine the maximum rate that the ventilation system must exhaust (e.g., cubic feet per minute (cfm)) for both emergency and non-emergency ventilation systems, and the supporting calculations for the ventilation system. The unexpected release of anhydrous ammonia or a loss of air circulation could result in the accumulation of ammonia vapors inside the machinery room exposing employees to fire, explosion, and inhalation hazards.
