29 CFR 1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: On or about December 8, 2021 employees at D&D Ingredient Distributors Inc. within the Flat Blending Building located at 5191 Kill Rd. Delphos OH 45833, were exposed to injury from pinch points created by moving components from Caterpillar unit 29 forklift (Model: CAT 2P3500 - Serial No. AT3440252). The employer did not ensure employees were trained in the purpose and function of the energy control program, with regards to powered industrial vehicle maintenance and repair, resulting in the employee becoming injured when the mast pulley chain pinched the employee's left hand.
