29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically identify procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: a) Die Cast Department -On or about November 15, 2021, the employer did not ensure that the established energy control procedures for the portable Regloplas Hot Oil machine, clearly outlined specific procedural steps for testing the machine to determine and verify the effectiveness of the lockout devices and other energy control measures applied for electrical and thermal energy sources, prior to initiating servicing and maintenance activities. The established procedure did not include steps for shutting down, isolating and securing the machine to control all hazardous energy on the Regloplas Hot Oil machine, including electrical, thermal, and chemical, procedural steps for the placement and removal of lockout/tagout devices, or specific requirements for testing the machine to determine and verify the effectiveness of the lockout devices and other energy control measures. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
