29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: (a) Facility at 322 Water Street in Warren, RI: On or about November 23, 2021, the employer did not provide adequate lock out/tag out training to ensure authorized employees were able to recognize applicable hazardous energy sources and the methods and means necessary for energy isolation to protect themselves and others from hazardous energy and amputation injuries while cleaning equipment, including, but not limited to, Ampco AL Series pumps, (Pump CJ#2).
