29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about November 11, 2021, the employer did not ensure an established procedure was followed for the application of energy controls (lockout/tagout) and were done in sequence during die set up on a hydraulic Cunningham Manufacturing Co. "C" style press, S/N 91597-214 with a 6" bore, 3" piston with ?' stroke. Employees did not apply energy controls by; turning off and/or shutting down the "C" style hydraulic swage presses [per the 1910.147(d)(2) requirements], isolating all energy sources [(d)(3)], affixing a lockout device to the energy isolating device [(d)(4)], and verify the equipment energy was isolated and de-energized prior to starting work [(d)(6)].
