29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition. (a) On or about November 16, 2021, and at times prior, at 410 South Railroad Avenue in Grace City, North Dakota, the employer did not regularly inspect walking-working surfaces for conditions that create or contribute to unsafe working conditions. The stairs on the elevating leg, and the loadout bin walkway had multiple holes over 2 inch by 2 inch in size creating trip and potential fall hazard from elevated working surfaces.
