29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about November 2, 2021, Hartung Glass Industries did not utilize lock-out/tag-out procedures for the Forvet CNC milling machine, serial # 423/08, by isolating the electrical and pneumatic power sources as required by the company's lockout tagout procedures, located in Hartung Industries Safety Program # 25, resulting in an employee suffering serious injuries while performing maintenance on the machine following a machine malfunction.
