29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Second Floor Production - To clear clumped up ingredients, employees removed the metal connector/chute between the ingredient hopper and feed pump on the Bulk 1 Gram NFD1000E Fruit and Ingredient Feeder, exposing the feed pump's rotating paddles/scrapers. Prior to removing the connector/chute, employees did not turn off the machine and did not lockout/tagout the electrical disconnect, which exposed employees to getting caught-in the feed pump's rotating paddles/scrapers, on or about November 3, 2021. No abatement certification or documentation required.
