29 CFR 1910.134(e)(2)(i): The employer did not identify a physician or other licensed health care professional (PLHCP) to perform medical evaluations using a medical questionnaire or an initial medical examination that obtained the same information as the medical questionnaire: a) Armstrong Center for Medicine and Health, Inc. located at One Nolte Drive, Kittanning, Pennsylvania 16201: On or about November 4, 2021, in the facility, the employer did not identify a physician or other licensed health care professional (PLHCP) to perform medical evaluations using a medical questionnaire or an initial medical examination. The employer required employees to wear respirators while providing care to suspected and confirmed positive COVID-19 patients.
