29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: Jobsite located at the CT Transit terminal, 100 Leibert Road, Hartford, Connecticut: From September 20, 2021 to November 3, 2021, an employee was required to enter approximately twenty one VS roof mounted units to perform preventative maintenance. The VS enclosure where the employee walked across and stood on an interior catwalk was classified as a permit required confined space due the potential fall hazard down into the interior of ductwork system. The employer had not developed and implemented procedures for employees to enter the space.
