29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. (a) Jobsite - On or about October 27, 2021 the employer did not ensure that lockout devices were affixed to the Scragg Edger when employees cleared jams or changed blades, exposing employees to amputation hazards.
