29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: MCCAUSLAND GREENHOUSE: On/or about October 21, 2021, the hazardous energy control lockout/tagout procedures were not written in step-by-step machine specifics for the employees during servicing and maintenance of equipment, such as (but not limited to) main panel/manual switch for the Bay 18 east in the McCausland greenhouse. On this day, the shirt sleeve of an employee replacing a bolt in the drive shaft at Bay 18 East in the McCausland greenhouse was caught in the drive shaft when the energy source had been turned back onto automatic and fell approximately 18 feet resulting in broken bones.
