29 CFR 1910.23(c)(10): The employer did not ensure that the top of a non-self-supporting ladder is placed so that both side rails are supported, unless the ladder is equipped with a single support attachment: a) Jobsite, parking lot area, 9365 McKnight Road, Pittsburgh, PA 15237: On or about October 12, 2021, an employee was using a 16-foot Werner brand aluminum extension ladder (model number: D1116-2) to paint a light pole. Both ends of the ladder side rails could not be supported by the pole because the pole was only approximately 4 inches in length and width. Per the manufacturer, the extension ladder was not designed for use with a pole lash, strap, or a v-rung.
