29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):(a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS) (IDENTIFY MACHINERY OR EQUIPMENT) a.) JN Pallet Company, LLC - On or about 9/20/21, employees did not lockout the pallet stringer when clearing wood jams. An employee removed a jammed stringer without conducting lockout/tagout and was injured by moving parts. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
