29 CFR 1910.134(a)(2): A respirator was not provided by the employer to each employee when such equipment was necessary to protect the health of the employee: Location - Domiciliary unit (DOM) at 200 Spring Rd., Bedford, MA, 01730 On or about September 2, 2021, the employer became aware that an employee tested positive for COVID-19. Co-workers who worked with this employee wore surgical masks and were not provided N95 respirators. Six employees tested positive resulting in two hospitalizations one of which resulted in the death of an employee.
