29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or have each affected employee use, the types of personal protective equipment (PPE) that would protect the affected employee from the hazards identified in the hazard assessment: 2 Northeast Blvd, Sterling, MA: 2 Northeast Blvd, Sterling, MA: On and before September 23, 2021 and continuing through at least February 18, 2022 , employees performing screen changes on extruder lines including line 16 were not using, and had not been provided, adequate leg protection to prevent burn injuries from fire and hot molten plastic spray. The employer's hazard assessment, which applied during this time period, did not identify adequate leg protection for employees who performed screen changes on extruder line 16. On September 23, 2021, an employee was severely burned while performing a screen change on extruder line 16 without adequate leg protection.
