29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). A) Jacob A. Weaver on or about August 05, 2021 at the address of 30777 Guys Mills Rd., Townville, Pa: An employee did not utilize lockout/tagout during servicing and maintenance of the machine. The employee did not lockout the machine before picking up a fallen board in and around the danger zone area. resulting in an amputation. An employee suffered a finger amputation.
