29 CFR 1910.502(f)(2)(i): a respirator to each employee and ensure that it is provided and used in accordance with � 1910.134 Generations at Lincoln, LLC, 500 Hall located at 811 West 2nd Street, Byron, Illinois: On or about August 10, 2021, tight fitting respirators were worn by employees in a way that interfered with the face to facepiece seal. An unvaccinated CNA (Certified Nursing Assistant) entered the room of a PUI (Person Under Investigation for COVID-19) wearing an N95 respirator with only one strap around the head, while the other strap was hanging, thereby reducing the protective effect of the N95 respirator. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
