29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: For employees working at the Sysco building in Palmetto Florida , employees were removing and replacing the drive belts on the two cooling tower fans when the kinetic energy was not isolated. The fans were spun by a single motor mounted between the fan assemblies. When one belt is removed from the pulley, the second fan still had the ability to continue move. This action creates an amputation hazard between the belts and the pulley, on or about July 12, 2021.
