29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). (a) On or about May 28, 2021 and thereafter, the energy isolating devices, as identified in the Lockout/Tagout Procedure ALC-LOP069, did not isolate the Wulftec Pallet Plastic Wrap Machine from the electrical energy source exposing employees to accidental/inadvertent restart of the equipment. (b) On or about May 28, 2021 and thereafter, the energy isolating devices, as identified in the Lockout/Tagout Procedure ALC-LOP072, did not isolate the Quest Tote Filling Conveyor Arm from the electrical energy source exposing employees to accidental/inadvertent restart of the equipment. (c) On or about May 28, 2021 and thereafter, the energy isolating devices, as identified in the Lockout/Tagout Procedure ALC-LOP075, did not isolate the Quest Pallet Conveyor from the electrical energy source exposing employees to accidental/inadvertent restart of the equipment.
