29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator or annually refitted: a.) Trinity Living Center, LP dba Quality Life Services-Grove City, located at 400 Hill Crest Avenue Grove City, PA: On or about June 11th, 2021, the employer did not provide a fit test to all employees required to wear N95 respirators. The employer required employees to wear N95 filtering facepiece respirators in areas of the facility where there were potential COVID-19 exposure hazards. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
