29 CFR 1926.503(c)(3): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: (a) Worksite- On or about May 22, 2021, the employer did not retrain employees when inadequacies in their knowledge of wearing personal fall arrest systems not tied off when they entered the Shaft "A" area to access a ladder. Employees were exposed to fall hazards.
