29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) On or about May 5, 2021, located at Section E and F, Rotary Feeder, employees were exposed to amputation hazards when performing service and maintenance tasks on equipment such as, but not limited to, the dust collector rotary valve without affixing a lockout device to the electrical disconnects.
