Text For Citation: 01 Item/Group: 001B Hazard:
29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about March 31, 2021, a Setup employee was exposed to hazards associated with the Weinig B22 Moulder Feeder (Company ID#2802, Machine No. 567-909) from the pinch points that were created by the feed rolls. The employer did not ensure that the Setup employee turned off and locked out the Weinig B22 Moulder Feeder prior to lifting the feed roll guard and making adjustments to the electric eye. b) On or about April 8, 2021, Moulder Feeder employee was exposed to hazards associated with the Weinig B22 Moulder Feeder (Company ID#2802, Machine No. 567-909) from the pinch points that were created by the feed rolls. The employer did not ensure that the Moulder Feeder employee turned off and locked out the Weinig B22 Moulder Feeder prior to lifting the feed roll guard and making adjustments to the electric eye. As a result, the remaining applicable energy control elements, involving machine or equipment [1910.147(d)(2)], machine isolation[1910.147(d)(3)], lockout/tagout application [1910.147(d)4)], dissipation of residual energy [1910.147(d)(5)(i)] and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing/maintenance activities.
