29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. First Choice Packaging, Inc. - Fremont, Ohio: On or about February 5, 2021, the employer did not ensure that each authorized employee affixed a lockout device(s) to energy isolating devices of the SencorpWhite Thermoforming machine (s/n 18-10499) in a sequential manner which included the application of lockout devices to control pneumatic and gravitational energy sources before employees performed a mold/form changeover. Employees that setup the machine were exposed to crushing injuries from the hazardous energy sources. b. First Choice Packaging, Inc. - Fremont, Ohio: On or about March 17, 2021, the employer did not ensure that each authorized employee affixed a lockout device to energy isolating devices of the SencorpWhite Thermoforming machine (s/n 18-10499) prior to adjusting the cutting anvils in the Trim Tool. Employees who adjusted the anvils were exposed crushing injuries from to the unexpected startup of the machine. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
