29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):(a) On or about 2/25/2021, the employer failed to ensure that lockout/tagout procedures were utilized in the following sequence for machinery including, but not limited to the Warner & Swasey Turret lathe #5: 1) Prepare for shutdown, 2) Machine or equipment shutdown, 3) Machine or equipment isolation, 4) Lockout/tagout device application, 5) Residual/stored energy release, and 6) Verification of isolation. Energy sources including, but not limited to electrical, mechanical, and pneumatic.
