29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. On or about February 25, 2021 during maintenance and repair activities, the employer did not ensure employees inspected an extension ladder prior to use. An employee used the top (fly) section without the base section attached to the extension ladder.
