29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition Employees engaged in work activities at the locations listed below, were exposed to trip and fall hazards in that protruding objects, unlevel surfaces, and ice were not maintained, causing employee falls to the same level resulting in injuries. A) Meat Freezer - inside freezer walking/working surface concrete is broken, leaving concrete surface unlevel with some concrete elevations approximately 4 and 1/2 inches in height. Metal plate leading to meat freezer door is unlevel with an elevation approximately 3 and 1/2 inches above concrete ramp. Ice accumulation below freezer on the west wall. B) Bakery / Deli Department - At the old mixer location on the South wall, rebar or concrete anchor bolts are protruding from floor causing trip hazards. The protruding objects are approximately 22 inches apart, and 1/2 inch in height. C) South register - Located on the floor, tile with a metal lip in a circle, has metal lip protruding approximately a 1/2 inch causing a trip hazard.
