29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: a) Area below Startup Cyclone. On or about February 21, 2021, employees performing servicing and maintenance, including but not limited to, clearing a clog in the cyclone. Annual periodic inspections of the energy control (lockout/tagout) procedures had not been conducted.
