At Oconto Falls Nursing and Rehab, LLC located at 100 East Highland Drive in Oconto Falls, Wisconsin: On or about December 21, 2020, the employer did not fit test employees prior to initial use of tight-fitting facepiece respirators. The employer required employees to wear respirators while providing care to suspected and confirmed COVID-19 positive residents.
