29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) In the facility workshop, on or about January 21, 2021 and at times prior thereto, lockout devices were not affixed to each energy isolating device on the drill collar roll machines (DCRMs) by authorized employees when Maintenance Technicians (MTs) and Maintenance Assemblers (MAs) connected and disconnected Maxwell MXW4 and MXW6 drill tools, exposing employees to caught-in and struck-by hazards.
