29 CFR 1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: Colonnade Senior Development, LLC dba The Colonnade Senior Living, 700 Weber Road, O'Fallon, IL 62269: On or about 2/28/2020, the employer had not provided medical evaluations to employees required to wear N95 filtering face piece respirators in the workplace. The employer required employees in the facility to wear an N95 filtering facepiece respirator when entering the rooms of known COVID-19 positive residents to administer care.
