29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): On or about January 20, 2021, and at times prior thereto, nightshift sanitation employees cleaning machinery in Bay 1 utilized lockout devices that did not indicate the identity of the authorized employees who applied the locks, exposing the authorized employees to the hazard of being caught-in moving parts.
