29 CFR 1926.952(a)(2): The employer did not ensure the employee in charge conducted a job briefing that met paragraphs (b), (c), and (d) of this section with the employees involved before they started each job: A.) United Electric Cooperative, Inc., Mayo Road, 41.123919, -79.136177, Corsica, PA, on or about January 8, 2021 and times prior thereto: An employee performing work operations including, but not limited to, connecting a guy wire to a strain rod of utility pole sustained electrical shock / electrical burns and subsequent amputation when coming into contact with an energized power line. The crew chief did not discuss special precautions associated with characteristics and condition including, but not limited to, the configuration of the hot phase and neutral locations during a job briefing. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
