29 CFR 1910.28(b)(7)(iv):(iv) Openings. The employer did not ensure that each employee on a walking-working surface near an opening, including one with a chute attached, where the inside bottom edge of the opening was less than 39 inches (99 cm) above that walking-working surface and the outside bottom edge of the opening is 4 feet (1.2 m) or more above a lower level was protected from falling by the use of personal fall arrest systems. On or about 11/03/2020, Fred Weber Reinforced Concrete Products, Inc. had not ensured employees working on or near a Besser, Model A-48 BiDi, concrete pipe manufacturing machine were protected from falling through a floor opening on the machine's rotating table.
