29 CFR 1910.265(c)(18)(i): Construction, operation, and maintenance of conveyors were not in accordance with American National Standard B20: a.) Collin Company, Kane Hardwood Division; sorter area -- Chain/Sprocket Associated with the Conveyor - - On or about 11/5/20, and at times prior thereto, chains and sprockets were not guarded to prevent employees from getting caught in them. An accident occurred in which an employee's foot was pulled into the sprocket area causing the employee to sustain a 2 toe amputation. One or more methods of effective machine guarding were not provided to protect employees in the conveyor area from hazards such as those created by ingoing nip points and rotating parts from chains and sprockets. In addition, the perimeter guarding was not so designed and constructed as to prevent the employees from being in the areas by the chain/sprocket in that employees could by-pass the perimeter guarding by utilizing two different stairways. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
