Text For Citation: 01 Item/Group: 001B Hazard:
29 CFR 1926.417(b): Equipment or circuits that are deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits could be energized: A.) B&B Designed Systems, Inc., 18746 Mill Street, Meadville, PA 16335, on or about October 9, 2020 and times prior thereto: An employee was attempting to land and terminate a ground wire onto the ground bus in panel CXPN-99. A ground wire touched an energized power circuit within the electrical panel resulting in an arc flash. The electrical panel was not de-energized and subsequently locked out or rendered inoperable prior to the start of the work operations. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
