29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) 6804 S. Ensenada Street, Aurora CO 80016: On or about October 5, 2020, and at times prior, the employer, DJV Concrete and Caisson Drilling, Inc. did not instruct employees in the recognition of unsafe conditions in that employees were not provided with a copy of the operator's manual. Employees were not provided with training to recognize and correct safety hazards presented by operating the auger drill in a manner restricted by the manufacturer. Employees were exposed to falling objects due to the Reedrill Texoma 600 Auger Drill not being used in accordance with the operator's manual. The feed arm was used to swing a hoisted caisson casing, contrary to guidance in the operator's manual.
