29 CFR 1910.147(c)(4)(i): Energy control procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about 9/17/2020, at the Brazosport Water Authority (BWA) facility located in Lake Jackson Texas, the employer did not ensure that energy control procedures or lock out and tag out (LOTO) procedures were developed and implemented for the following equipment but not limited to: a. Millennium III C250 VF Chlorine Dioxide Generator Re-circulate pump AP-1 for recirculate vessel V1- BWA System #1 (or C31010). b. Millennium III C250 VF Chlorine Dioxide Generator Re-circulate pump AP-1 for recirculate vessel V1- BWA System #2 (or C31011).
